The Fine Print.
Provider will hold and maintain a valid, unrestricted and unrevoked license in good-standing to practice their profession in the state(s) that Provider has previously disclosed their being licensed in.
Provider represents that it has the qualifications and will maintain them in good standing during the term of this Agreement. And Provider agrees that:
Provider is not now, and has never been, excluded or suspended from, participations in, or sanctioned by, any federal or state health care program, including Medicare and Medicaid, or convicted of a criminal offense related to (i) the delivery of or billing for health care services, or (ii) the neglect or abuse of clients.
Provider will abide by all applicable governmental laws, rules and regulations. In addition, Provider will abide by all policies, rules, regulations, and guidelines of ESAD Int'l pertaining to providers having comparable duties, as such may be amended at ESAD’s sole discretion, and such amendments will not affect the enforceability or terms of this Agreement.
Provider will notify ESAD Int'l within 24-hours of any material change in the status of the qualifications of Provider, including, but not limited to any alleged suspension or suspected or known investigation regarding Provider’s license(s), privilege(s) and/or certification(s). If Provider ceases or violates the requirements of this Section, ESAD Int'l may on written notice to Provider terminate this Agreement.
Provider has not been convicted of a criminal offense and has not been named as a defendant in any criminal or civil action or proceeding alleging (i) violation of any state or federal statute or regulation governing discrimination or sexual harassment, or (ii) any tort for assault or battery related to any alleged harmful or offensive touching of another person.
If ESAD Int'l incurs any liability due to Provider’s failure to conform, Provider agrees that any such liability will constitute an act or omission by Provider.

ESAD-Int'l (ESADoggy) is seeking forward-thinking mental/medical healthcare providers who are interested in improving clients’ quality of life through a broad array of condition qualifiers.
We build long-term professional relationships. Our first three therapists brought on board, over five years ago, are still on our team. And we're really a better place thanks to them.
Our industry-leading compensation package.
This opportunity requires you to conduct client mental health assessments, disability verification (multiple hour long sessions) -- and possibly issuing an Emotional Support Animal or psychiatric service dog letter of recommendation (we will supply the fill-in-the-blanks ESA letters).
We do not compensate for letter writing, as that's illegal in many states now. You're paid for your clinical and administrative time.
Depending on your experience, our clinicians earn for $60-85/hr.

Everyone is busy. Everyone gets lots of email. If we have to chase you, we’re going to cut you loose. Twenty-four hours is the maximum you should take before you respond, even it is a simple acknowledgement. Then you can establish a reasonable time to respond and manage expectations.
We appreciate the occasional mulligan, so we have a one-time tolerance for a missed event. The second time? There is no second time.
